Getting an NHS pilot: what a founder can do before chasing a framework

A pilot is not a procurement route. How to use NHS England's supplier and innovation pathways to validate a problem before you spend months on framework applications.

The most common healthtech mistake is treating a friendly pilot as evidence of a route to purchase. Pilots test whether the problem is real. Frameworks are how money moves. They are different projects.

Key takeaways

  • NHS England publishes supplier guidance for organisations selling into the NHS.
  • The NHS Innovation Service and local Health Innovation Networks exist to route innovators to support.
  • Frameworks are a common procurement route, and recommended framework guidance exists for digital and IT.
  • A clinician saying 'this is useful' is validation, not a budget.
  • Find the budget holder and the buying route before scaling a pilot.

Two separate questions

Question one: does this solve a problem a trust actually has, in a way clinicians will use under real conditions? Question two: who holds the budget, under which contract vehicle, and what evidence do they need to sign?

Founders often spend a year answering question one and assume question two follows. It does not.

Use the published pathways

NHS England's 'supplying to the NHS' pages set out how suppliers engage, and its innovation pages point innovators to the NHS Innovation Service and to local Health Innovation Networks that can help route you to the right people regionally. The procurement pages and the central commercial function's framework strategy recommendations explain how frameworks fit into buying, including for digital and IT.

Before you chase a framework

A short readiness list.

  • Name the trust problem in the trust's own language, with a measurable outcome.
  • Identify the budget holder, not just the enthusiastic clinician.
  • Confirm what information governance, clinical safety and IT assurance will be required.
  • Get one written statement of the value delivered in a pilot, quantified.
  • Check whether an existing framework already covers your category before applying to a new one.

What to ask other founders

Guidance tells you the rules. Other founders tell you what actually happened. These are worth asking as a structured Advice or Critique request.

  • Who held the budget for your first NHS pilot, and how did you find them?
  • How long from first conversation to money moving?
  • Which framework, if any, did you end up on?
